well i write it this way

bass/right hand chords

and here is how i write out chords

C#= C sharp
Cb= C flat
C#major chord        or    C#maj
C# minor chord       or     C#minor


to write a single note, you can just put in Eb (for example).

C/ C major chord
F/ F minor chord
G/ G major chord
A/ G-A-C-E
C/ C major 7

that is how you can write out chords. Just put the bass note (playing it with left hand) on the left and put right hand chords on right.
If you want the left hand to play more than one key at the same time, you can put it like this:

C-E/ C major chord              (Put in C-E).

I put dashes to indicate the keys that you play together as a chord. I will put a comma to indicate the keys that you play seperately.

C,E,F/ C, E, F


churchboy i didn't know what you meant by (2nd) or (3rd). Can you explain?

2nd and 3rd inversions. Note these are not my chords i just posted them to share them. Im trying to write the chords out as Eb (G-Bb-Eb)

Ok good. Just wondering. The one you just posted is kind of confusing, especially the "Abadd9" part. To me, instead of putting 2nd or 3rd inversions, they should write out the notes in order the way how it suppose to be played.

For example, in the key of G

instead of writing G(2nd inversion) they can just write B-D-G

i know but i had to be greatful for them giving me the chords like they are rather than nothing at all. I would have wrote it that way myself.

you see i never heard of this song and the way how they wrote it out was Ebm, so i assume it was Ebmajor.
