The P&W leader does the singing and helps out the music director in the vocal areas. While the music director is in charge of the music roles.

Well usually the...

Praise and worship leader is in charge of the praise team and leading praise and worship, that's it.

Music director is in charge of the musicians and the instrumental part of the music, that's it.

But the Minister of music is in charge of it all. They could be a hands on m.o.m. (direct the choir, play, lead praise and worship) or a hands off m.o.m. who chooses not to do all of that but still chooses the songs, hire and fire music staff, does the pay and make sure that the music ministry is producing good music.

Yes, If both positions are Pastorally ordered.  Now, they do operate in separate functions.  I agree that the music director should cover the areas and duties of the instrumental aspect of the music ministry or department.  But please be mindful of the authority of the P&W leader for they move by the assignment of the Pastor(s).  Overall, they both should serve together as one sound, because after all it is all about CHRIST...Right!!!

I've been in a situation earlier in my life (about two years ago) where I was the Min of Music but the co-pastor of the church was the worship leader.

As you can tell by my use of the past tense, IT DIDN'T WORK!

She thought that she could pull the (the Pastor's my husband and I don't have to come to rehearsal but I'm leading all of the songs anyway as a matter a fact, if I'm not there don't teach any new songs) I'm the CO-PASTOR card out anytime things didn't go her way.

I think the only time a M.D. and a Worship Leader can co-exist is if they have a Minister of Music to.

If the Worship Leader's the MOM but don't know how to play an instrument, they could quite possibly select songs too difficult for the band. If the MD is the MOM there's a chance that the opposite could happen.

The answer is because we're wired differently...

That's bad when people in leadership pull that authority card in the wrong place. We should all respect our leaders but man!! I couldn't imagine. I think it could be done but my question is why would someone want to where all of those hats if there are capable people to assist you in Ministry.

Jethro told Moses to select some help before he went crazy. Typically people who like to hold dual roles when they don't have to are filled with the "me" spirit. Some people live for there spotlight. Some of em already got their performance predetermined. It's a tough situation to be in. It damages the people that God could be using. Futhermore that end up somewhere else!!!
